Use less paper

To reduce the amount of paperwork in our practices and to make life easier for our patients, we’re introducing electronic forms for you to fill in ahead of your appointments. These will replace many of the forms you normally complete in practice.

The forms are easy to complete on any smartphone, tablet or computer. But if for any reason you can’t fill the forms in online don’t worry, we’ll still have some paper copies available in practice should you need them.

The forms you will start to receive are:

Covid-19 Screening Form

This form will provide information to the practice about any exposure to, or symptoms you may have, in relation to Covid-19.

The Medical History Form

This form will provide information to the practice about any changes to your medical history that your dentist needs to be aware of.

What to expect

You will receive a text message from mydentist prior to your appointment. This text will look similar to:

Hi, your mydentist practice is open. Your dental appointment is at mydentist, Stevenson Square, Manchester on 27/04/2021 at 13:15. Please access this link {Link} to update your forms online before you visit the practice.

When you click on the link, you will be directed to a website page showing your name and the time of your appointment. There will also be a list of forms that you need to complete. Please fill out the forms allocated to you.

When you have finished just press "complete" to send them back to us.

Why are we doing this?

We are always looking for ways to improve your experience. Moving to online forms means that:

  1. There’s less paperwork to handle, keeping our patients and staff safe and protected during Covid-19.
  2. You can relax – you can complete the forms in your own time and there’s nothing to fill in when you arrive.
  3. You have a safe and secure way to share your details with the practice.
  4. Your data will be accurate and ready at the practice when you arrive.
  5. Your visit to the practice can be shorter and more convenient.

What if the practice doesn't have my mobile number?

Next time you come in for an appointment you can get your number added onto the system. You can also give us a call, although please do bear in mind that our phone lines are busier than usual at the moment.

What if I make a mistake on the forms?

If you make a mistake, don't worry.

  • If the mistake is on your Covid-19 screening form or something has changed then please call the practice to confirm before attending.
  • If the mistake is on your Medical History form then please inform reception or your nurse on arrival at the practice and they will be able to resolve anything for you. This might involve completing a paper copy if required.

Since I filled in the form I developed Covid-19 symptoms, what should I do?

Please phone the practice to let them know. Do not attend the practice until you have spoken with a member of our staff on the phone. Bear in mind that our phone lines are busier than usual at the moment.

What if I don't want to or can’t fill my forms in electronically?

That's not a problem. You are still able to complete all forms on paper at the practice. Please do ensure that you allow extra time prior to your appointment to complete this.

How do you handle my data?

The website page where you complete the online forms is managed by our technology partner PatientComms. You can find out more about the data we hold and how we process it via our privacy policy.

You are able to opt out by calling our practice or letting the team know when you book your appointment over the phone.
